Hi there, I've lived in various places round Italy and am moving back to Milan permanently now. Vino-rosso is right the traffic is a nightmare, but only during rush-hour (there are the usual am and pm, plus a 3rd one from 11pm-1am when everyone is out and about). Or you could just get a scooter and whizz round on that, it's much easier for parking too.
Milan is not, however, a gigantic city. In fact the city centre is pretty small and it's easy to get around on foot. Public transport links aren't at all bad and very reasonably priced. It has lots of periferal towns that are sort of merging into suburbs, which may give the impression that it is huge. The Milanese are generally known for being unfriendly and snobby, but once you get to know them (and they get to know you) they are incredibly warm and generous.
My main tip: learn Italian. In fact, start learning now so you're getting the hang of it by the time you get over here, you'll find it a huge help. Let me know if you have any specific questions.
