What is the Italian attitude to cold calling on companies, that is just turning up at the premises and asking to see a relevant person to put forward a proposal for a business idea? Many years ago I was told the correct way was to send a letter of introduction and request an appointment, is this correct?

You can send an email but do not expect an answer, Italian are horrible at email ethic.
You could however send an email and then call telling them you send an email - To be honest I would call and ask whom you could speak with who can help with your question. I am not sure anymore about the letter either since there is junk mail in Italy too now - I would just call
